If you have supplementary insurance, treatments will be billed directly to your insurance company. For basic insurance only or outside the supplementary package, the above rate applies and you will receive a bill. This note is subject to payment terms.<\/p>\n<p>In case of repeated rejections of the submitted claim, by your health insurance company, we are forced to send you an invoice.<\/p>\n<p>If you are under treatment at our practice, you agree to participate in a customer satisfaction survey through Qualiview (imposed by health insurance companies). You agree to the house rules and you have read the privacy policy. In addition, you agree that we share data from the electronic patient record with the national physical therapy data (LDF).
